For all your general dental care needs, we will do our best to assist. Please contact us at (808) 949-2908 to schedule an appointment or contact us using our web form. Our schedule varies each week, but office hours are from 8:00 AM to 5:00 PM on weekdays and 8:00 AM to 1:00 PM on some Saturdays.

Your Visit

Prior to your appointment, we ask that you confirm your appointment through our automated text message reminder system or by calling our office at (808) 949-2908. In an effort to best serve all of our patients, we require appointment confirmation at least 24 hours before the appointment. Should your schedule change and you are unable to make your appointment, please call our office as soon as possible.

If you are visiting our office for the first time, thank you for choosing Dr. Suehiro! We know you have many options for dental care in Hawai‘i and we value your trust in us.

Prior to your appointment, please fill out the new patient forms linked below. You must have a scheduled appointment to access the link. If you prefer to print the forms out, let us know and we can provide you with digital copies. You can email completed digital forms to or fax printed forms to (808) 951-7087. If you are unable to complete the forms prior to your visit, they will be provided to you at the office.

Please arrive 15 minutes prior to your scheduled appointment to complete your new patient registration and familiarize yourself with our office.

Financial Policy

Upon completion of your treatment, we will submit the insurance reimbursement forms. Co-payments will also be collected at that time. We can provide you with an estimate and/or submit a pre-authorization to your insurance prior to treatment, upon request.

We accept cash, checks, and all major credit cards.

Your insurance plan is a contract between you/your employer and the insurance carrier. Although we are familiar with many of the plans, you are responsible for your account and should direct coverage questions to your employer/HR department or carrier. 

We ask that you provide our office with your insurance carrier, policy number, and, when possible, information regarding your coverage. Any changes to your dental insurance policy and personal information that may affect said coverage should be communicated to us in a timely manner. Please also inform us if you have more than one insurance plan so we can optimize your coverage.

As a courtesy to our patients, our office will submit your claims to your insurance carrier. When possible, we offer electronic submission (EDI) to expedite the claim process. Most payments from our local carriers are processed within a week of receiving the claim.

When planning extensive treatment, we will send a detailed estimate or pre-authorization to your insurance carrier(s). In most cases, the response is sent to the insured within 2-3 weeks. Please note that, for privacy, your insurance may inform and communicate with you as its subscriber, and not with our office.
